Kentucky Inspection Sticker

I'm looking for anyone that has an original Kentucky inspection sticker from the late '60s or early '70s that could post a picture or tell me exactly what is printed on the back side. I would greatly appreciate any help.

I've been to that web site and they are not correct for the years I am looking for. I'm hoping someone will have one on an old car setting somewhere or even have a new one that was never placed on a vehicle.
